The best way we have of telling is breathing changes. Patients can have Cheyne Stokes respirations (alternating deep slow breathing, not breathing, and then rapid shallow breaths) for days or weeks. Patients can gurgle for days (essentially once they lose their ability to swallow - so all the spit and mucus we swallow, they can’t and so you can hear it as they breathe.
